Home Health Services Skilled nursing, home health aides, physical, occupational, and speech therapies, and medical social work services are delivered in the privacy and comfort of the patient’s home. Our skilled professionals and staff undergo a comprehensive orientation and competency program that will ensure that only the best and most caring individuals are chosen to deliver care. Inter-Active Home Health has two distinct and separate types of licenses and provides different types of care. Hospice Service Our Hospice Program provides specialized supportive services in the home to clients diagnosed with terminal illnesses who no longer wish to receive treatment designed to cure or to prolong life. The goal is to assist the client to achieve a peaceful and comfortable end. The hospice philosophy encourages clients and their caregivers to live each day fully. The focus is on improving or maintaining the quality of life of the clients and their caregivers while the terminal illness progresses. Well Mom-Well Baby Our Well Mom-Well Baby program provides specialized services to mothers and babies during the prenatal and post-partum stages at home. Prenatal services include care of high-risk pregnancies. The goal is to assist the mothers to learn the proper care for their babies and identify, at the onset, any possible problems. Infusion Service Close coordination between our nursing divisions and in-house pharmacy (InterCare Pharmacy) provides optimal outpatient intravenous (IV) therapies for hydration, parenteral nutrition and antibiotic infusion purposes. Pharmacy & Home Medical Equipment (HME) InterCare Pharmacy, our full service in-house pharmacy, coordinates infusion management with our nursing services to assure that uninterrupted and optimal care is delivered to the patient. Our State Board licensed pharmacists promptly respond to our patients’ prescribed medications. Our HME personnel are ready to deliver patient equipment and take the time to explain safe use, care and maintenance of the equipment. We take care of all necessary billing of Medicare and other payors for the patient. Personal Assistant Service & Private Duty We provide personalized one-on-one hands-on care through qualified and trained certified nursing assistants and home health aids. Sitter Service is helpful for elderly clients who are sick, weak, functionally dependent, or are confused and agitated, and in need of someone to watch over them to prevent harm or injury.
